Step 2: Complete the quick eligibility survey

Step 3: If approved, you'll receive a letter of medical necessity (LMN)

Step 4: Purchase the Levels membership that works for you (pricing & plans here)

Step 5: Submit a reimbursement claim to your HSA or FSA administrator

What is TrueMed?

What if I already purchased Levels?

If you already purchased Levels and want to request reimbursement from your HSA or FSA:

Step 2: Complete the survey to get your Letter of Medical Necessity

Step 3: Request an itemized receipt from Levels support

Step 4: Submit both documents to your HSA or FSA administrator for reimbursement
