Levels Labs Policies
Overview
This article provides an overview of the Levels Labs policies.
Results Access
Once Levels receives your Labs results, your results will be uploaded to the Levels app and your dashboard in the member portal for your review. Regardless of your membership status, you will be able to view your Labs results in the Levels app, along with your historical glucose and log data.
Refunds
- Levels members can request a full refund for unused Labs order within a year of purchase.
- Refunds will not be issued after one year or once you’ve completed your Labs appointment.
- If you request a refund on your membership and you have already used/redeemed your lab order, your membership cost will be refunded, less the cost of the lab order.
- The Basic Panel is included in the cost of the annual membership ($199). We are not able to issue a refund for the Basic Panel included in the membership, nor can we prorate the membership cost if you do not want to or cannot use the Basic Panel.
- Labs orders expire after one year and must be used within one year of purchase.
Ordering and Results Overview
Levels offers a Labs feature that enables members to request lab testing through a structured, third-party integration. This feature is made possible through the coordinated services of several independent partners:
- Lab orders are created by a licensed physician from an independent physician network facilitated by MD Integrations.
- The order is routed via our partner platform, Healthie, and transmitted to Quest Diagnostics, a national clinical laboratory.
- Quest performs the requested testing and returns the lab results.
- These results are then made available to you within the Levels app exclusively at your request / on your behalf and for your personal use.
Levels does not create or process laboratory orders, nor does it provide medical evaluation, clinical interpretation, or diagnosis. Levels is not a healthcare provider and does not maintain a clinical relationship with you. Except as required by applicable regulations concerning critical or life-threatening values (as described below), your lab results are not reviewed by a physician as part of this process.
Your use of this feature is entirely at your direction, and Levels displays results solely for informational purposes. You are responsible for reviewing your results and seeking medical advice from your own physician or licensed healthcare provider. Do not rely on the Levels app or any information presented through this feature as a substitute for professional medical evaluation or care.
In rare cases, if your results include values that are flagged by Quest or the ordering physician as critical (per federal and state laboratory safety regulations), you may be contacted directly by a healthcare professional as required. Levels itself does not provide medical follow-up or emergency support. If you are experiencing a medical emergency or have concerns about your health, call 911 or contact your personal healthcare provider immediately.
