What do I need to schedule my appointment?


Answer:

Scheduling your lab appointment is simple, but there are important things you’ll want to have ready to make it seamless, including:

Basic demographic information (name, date of birth, etc.)

A sense of your availability to choose a time that works for you

Credit Card information when you purchase Levels Labs through us

💡  Important Notes:

  • The information you provide during scheduling must match your ID at the time of your appointment to ensure you can be tested.
  • Be sure to have quick access to the PDF of your lab order at the time of your appointment. This PDF will be sent to your email and will also be available in your Levels Labs dashboard. You don’t need to print this PDF—occasionally Quest has issues with electronic lab orders, and if that is the case you’ll need to show your lab order PDF.
