What to Bring to Your Lab Appointment

To ensure a smooth lab visit, bring the items below with you.

What to bring

• A valid photo ID
  • Your identification must match the information you provided when scheduling.
• Quick access to the PDF of your lab order
  • This PDF is sent to your email.
  • It’s also available in your Levels Labs dashboard.
  • You do not need to print it.
  • Occasionally, Quest may have issues with electronic lab orders. If that happens, you may be asked to show the lab order PDF.
• A water bottle
  • Staying hydrated can make blood draws easier.

Important reminders

• Fasting may be required for some tests. Review the pre-appointment instructions we emailed you.
• Arrive on time to keep your appointment running smoothly.
