Levels Labs (Basic and Comprehensive panels) can be purchased in all 50 U.S. states. Lab visits are completed at Quest Diagnostics locations nationwide.

You can purchase Levels Labs and complete your lab visit at a local Quest location.

Travel required for lab visit

You can purchase Levels Labs in the locations below, but you’ll need to visit a Quest location in another state or territory to complete your lab work due to limited facilities:

• Hawaii (HI)
• North Dakota (ND)
• Rhode Island (RI)
• South Dakota (SD)
• West Virginia (WV)
• Wyoming (WY)
• American Samoa (AS)
• Guam (GU)
• Puerto Rico (PR)
• U.S. Virgin Islands (VI)

Before you purchase

• Lab orders are valid for one year from purchase.
• You’ll need an active U.S. address and phone number.

Refunds and expirations

• You may request a full refund for unused Labs orders within one year of purchase.
• Refunds are not issued after one year, or once you’ve completed your Labs appointment.
• Labs orders expire after one year and must be used within one year of purchase.
