Where is Levels Labs available?

Levels Labs (Basic and Comprehensive panels) can be purchased in all 50 U.S. states. Lab visits are completed at Quest Diagnostics locations nationwide.

✅ Most states: Purchase available with travel to local Quest locations.

🗺️ Travel required for lab visit: You can purchase Levels Labs in these states and territories, but you'll need to visit a Quest location in another available location to complete your lab work due to lack of facilities available:

• Hawaii (HI) 
• North Dakota (ND)
• Rhode Island (RI)
• South Dakota (SD)
• West Virginia (WV)
• Wyoming (WY)
• American Samoa (AS)
• Guam (GU) 
• Puerto Rico (PR)
• U.S. Virgin Islands (VI)

• Lab orders are valid for one year from purchase
• Requires active U.S. address and phone number
