Where is Levels Labs available?

Answer:

Levels Labs is available in most U.S. locations, with a few exceptions. We don’t currently offer Levels Labs in:

🚫 Arizona (AZ)

🚫 Hawaii (HI)

🚫 New Jersey (NJ)

🚫 New York (NY)

🚫 Puerto Rico (PR)

🚫 Rhode Island (RI)

At this time, only the Basic Panel is available in the following states:

• Alaska (AK)
• Connecticut (CT)
• Massachusetts (MA)
• New Hampshire (NH)

Additionally, although we offer Levels Labs for purchase in the following states, there are no Quest Diagnostics locations directly in these states:

• North Dakota (ND)
• South Dakota (SD)
• Wyoming (WY)
• West Virginia (WV)
• ⁉️ If you live in one of the above states, you may travel to a neighboring state to complete your labs:
  • Note: Lab orders are valid for one year. You may request a refund for unused lab credits that were not included for free as part of a membership within a year of purchase. View our full refund policy here.

💡 Important Notes:

• Levels members who purchase Levels Labs must have an active U.S. address and phone number.
• Lab visits are available at any of Quest Diagnostics 2,000+ locations across the United States.
• If you live in AK, CT, MA, or NH, the Basic Panel is the currently available panel. Stay tuned for future updates regarding additional options in these states!
• If you live in ND, SD, WY, or WV, there are no Quest Diagnostics locations in these states, but you may travel to a neighboring state to complete your labs. Lab credits are valid for one year. View our full refund policy here.
