Where Levels Labs is Available
Levels Labs (Basic and Comprehensive panels) can be purchased in all 50 U.S. states. Lab visits are completed through the lab partner assigned to your order.
Lab partners
Each Levels Labs order is tied to a specific lab partner. Your lab draw must be completed at a location that matches the partner on your order.
- Quest Diagnostics — Used for most orders. 2,000+ locations nationwide.
- BioReference Laboratories — Used for orders in New York and New Jersey. Walk-in only (no scheduled appointment time).
- LabCorp — Used for certain historical orders only. Your order details/SKU will indicate if your order is through LabCorp.
Where Levels Labs is available
Most states
You can purchase Levels Labs and complete your lab visit at a local Quest location.
New York and New Jersey
Labs in NY and NJ are fulfilled through BioReference Laboratories. These locations are walk-in only and will not have a set appointment time.
Travel required for lab visit
You can purchase Levels Labs in the locations below, but you'll need to visit a Quest location in another state or territory to complete your lab work due to limited facilities:
- Hawaii (HI)
- North Dakota (ND)
- Rhode Island (RI)
- South Dakota (SD)
- West Virginia (WV)
- Wyoming (WY)
- American Samoa (AS)
- Guam (GU)
- U.S. Virgin Islands (VI)
Before you purchase
- Lab orders are valid for one year from purchase.
- You'll need an active U.S. address and phone number.
Refunds and expirations
- You may request a full refund for unused Labs orders within one year of purchase.
- Refunds are not issued after one year, or once you've completed your Labs appointment.
- Labs orders expire after one year and must be used within one year of purchase.
