Where Levels Labs is Available

Levels Labs (Basic and Comprehensive panels) can be purchased in all 50 U.S. states. Lab visits are completed at Quest Diagnostics locations nationwide.

Most states

• Purchase is available with travel to local Quest locations.

Travel required for lab visit

You can purchase Levels Labs in these states and territories, but you’ll need to visit a Quest location in another available location to complete your lab work due to limited facilities:

• Hawaii (HI)
• North Dakota (ND)
• Rhode Island (RI)
• South Dakota (SD)
• West Virginia (WV)
• Wyoming (WY)
• American Samoa (AS)
• Guam (GU)
• Puerto Rico (PR)
• U.S. Virgin Islands (VI)

Before you purchase

• Lab orders are valid for one year from purchase.
• Requires an active U.S. address and phone number.

Refunds and expirations

• You may request a full refund for unused Labs orders within one year of purchase.
• Refunds are not issued after one year or once you’ve completed your Labs appointment.
• Labs orders expire after one year and must be used within one year of purchase.
