How to Find Your Lab Order
Typically, you won’t need your lab order slip for your Quest appointment. Your appointment confirmation ID is usually sufficient.
However, if you want to locate your lab order slip (also called a requisition form), you can find it in two ways.
Option 1: Find the requisition email
Look for an email titled “Levels Labs: Your Requisition Order Form”. This email includes a downloadable copy of your requisition form.
Option 2: Download it from the Member Portal
- Log in to your member portal from a desktop browser.
- In the left navigation panel, select My Labs.
- On the right-hand side of the page, download the PDF of your lab order requisition form.
Before your appointment
Before heading to your Quest Diagnostics appointment, make sure you have:
- A government-issued photo ID
- Your Quest appointment confirmation ID (look for an email titled “You’re all set—here are your Lab appointment details”)
