How do I complete my CGM order after purchasing a CGM kit?

Are there any steps that I need to take after purchasing a CGM to receive my order?

🛑✋ Please Note: If you ordered a CGM through Levels, you must complete a few additional steps in order for us to process your CGM order.

Steps to Complete:

After ordering a CGM through Levels, you will receive an email with the subject line 'Action required to complete your Levels order'
Open this email and select the green 'Click here to select your sensor' button
Complete the required steps to select your CGM sensor type
Wait for final confirmation via email that your CGM order is being processed

About the CGM sensors:

Levels currently offer two types of CGM sensors—the Dexcom G7 and the Freestyle Libre
The CGMs are the same cost ($184 per kit), and each CGM kit provides you with 1 month's worth of sensors for continuous glucose monitoring
The Dexcom G7 is offered as a part of our observational research study
- 💡If you select the Dexcom G7, you will be asked to provide your consent through our informed consent form
- Study participation is optional, so if you prefer not to be a part of the study, you can select the Freestyle Libre sensor, 'Bring your own Sensor', or use Levels without a CGM
- Learn more about eligibility criteria and study enrollment
The Freestyle Libre is offered on a prescription basis that Levels helps to facilitate with collaborating physicians
- 💡If you select the Freestyle Libre, you will be asked to complete our medical consult form and identity verification process
