Your lab appointment will take place at one of 2,000+ Quest Diagnostics locations across the U.S. Most members can find a Quest location within 10 miles of home.

The specific location depends on what you select during scheduling. You’ll receive a confirmation email with the address.

Schedule through the Levels Member Portal

Your appointment must be scheduled through the member portal. Our scheduling flow shows the Quest Diagnostics locations that are available to process Levels Labs orders.

• If a Quest location is not shown in our scheduling flow, but is shown elsewhere online, that location is not compatible with Levels Labs.

Important notes

• If you live in HI, ND, RI, SD, WV, WY, AS, GU, PR, or VI, you may need to travel to another state due to limited facilities.
• Lab orders are valid for one year from purchase.
